Community nursing staff care for people in their own homes or in home environments. It is a highly skilled role which promotes health, avoids hospital admission and allows person-centered care in the place of the patient’s choosing. There are career paths in District Nursing, Care Home Support team and Care Home Nursing within Midlothian partnership’s own care home.
Care home nursing and District Nursing provide nursing care to people in their home, this ranges from long Term health condition management, wound care, nutritional needs, and medication management to End of life care – this is not an exhaustive list. All staff are supported to develop and learn new skills and there is also funding for courses in advanced practice as well as the Specialist Practitioner District Nurse qualification pathway.
