Community Nursing

Intensive Home Treatment Team
The Intensive Home Treatment Team (IHTT) works in partnership with the patient, their family, and carers in their treatment of and recovery process from an acute episode of mental disorder requiring intensive input.  This service is delivered in the patient’s own home, in clinic or other community resource as a direct alternative to inpatient treatment where safe to do so

The team operates an assertive outreach model of care by offering a range of alternatives for patients whose presentation may be considered for hospital admission.

They respond rapidly to referrals, offering triage, initial assessment and a holistic approach to care planning and treatment. They may offer a short-term intervention until the crisis has resolved, symptoms have stabilised and coordinate follow up appropriate support thereafter.

Community Mental Health Team

The Community Mental Health Team (CMHT) are a multi-disciplinary team of mental health professionals, employed by Midlothian Health & Social Care Partnership, with a range of skills and areas of expertise.

We provide assessment, care planning and treatment, and coordinate multi agency support and follow up, for people aged 18-64 in Midlothian with moderate to severe mental health difficulties.

Referrals are received from GPs and we work closely with colleagues in the IHTT, Psychological Therapies Service, Substance Use, and inpatient unit at the Royal Edinburgh Hospital to coordinate most of the outpatient care.

We operate a key worker system to empower service users to participate in their ongoing treatment and care.
