- People’s Homes
- GP surgery
- Nursing homes
- Our Community Hospital
Adopting a Home First approach, we deliver nursing care and treatment to people in our community:
- with a wide range of long-term conditions
- to keep people safe and well, living in their own homes or a homely setting
- to promote self-management of long-term conditions and independence
- to provide care in the most appropriate environment for the individual such as people’s homes, GP surgeries, care homes, nursing homes and our community hospital
- to aid recovery following short term illness or on discharge from a hospital
WLHSCP Community Nursing Services incorporates a wide range of nursing services which all work alongside and complement each other:
- Bladder and Bowel Service
- Care Home Team
- Community Treatment and Care
- District Nursing
- Hospital at Home
- Tippethill Hospital
- Vaccination Team

Continuity of care for your team’s patient caseload, supports the building of professional relationships with patients/families/carers that you care for on a regular basis.
West Lothian Community Nursing working hours are:
Bladder and Bowel Team : 0830-1630 Monday to Friday
GP surgery based CTAC team: 08:30-16:30 Monday to Friday
GP surgery based District Nursing Teams: 0830-1630 Monday to Sunday
St. John’s Hospital based Out of Hours District Nursing Team: 16.00-midnight Monday to Sunday.
St. John’s Hospital based Hospital at Home Team: 0830-1630 Monday to Sunday
Tippethill Hospital: 24 hours care provision
GP surgery based Vaccination Team: 0830-1630 Monday to Sunday
