The learning disability service (LDS) is currently undergoing significant redesign, providing broad range of opportunities to systematically remodel our clinical environments, and our patient pathways across primary care, community based and inpatient facilities.
Current provision is set across –
Community Learning Disability Teams – x 7 – geographically aligned
Specialist Teams – x 2 – Epilepsy and Acute Hospital Liaison
Intermediate Teams – x 3 – Mental Health, Forensic and Positive Behavioural Support
Inpatient Facilities – x 4
- Islay Centre – 10 rooms located on the Royal Edinburgh Hospital site, for autism / PBS
- William Fraser Centre – 12 rooms located on the Royal Edinburgh Hospital site, for Mental Health / Forensic
- Glenlomond – 5 rooms located on the Royal Edinburgh Hospital site, for Forensics
- Camus Tigh – 6 rooms located in Broxburn, for Mental Health / Forensic
Key function of the LDS is to promote and enable the best health status possible for people with learning disabilities. All models of care are based on sound clinical evidence, are human rights and least restrictive focused to enable positive person centred outcomes.
Achieving health equality is key, with robust assessments informing a clinical formulation of appropriate interventions, a sustainable support plan and a relapse plan all of which will pick up on the interaction and complexity of physical and mental health, and social and behavioural presentations.
